New Patients

We are currently accepting newly referred patients and are among the Edmonton pediatricians accepting new patients. 


Referral information: 

We are a group of consulting pediatric specialist doctors. Our care model is that all children should have a family physician or nurse practitioner who serves their primary care needs. We see children on a consulting basis and then return their care to their primary care provider. While we do see children with complex health needs or social situations regularly, we still ask them to have a primary care provider to ensure their health needs are met comprehensively. 


Currently, we are accepting new patients by referral only, specifically from physicians or nurse practitioners. Referral wait times can vary from 1-12 months and are triaged based on medical urgency. We only accept referrals if there is a clinical question or a need for comprehensive complex care coordination. Please note, we are not able to accept referrals for healthy children requiring checkups or primary care. Additionally, we do not accept transfer-of-care requests. 


If your pediatrician is retiring, or if you are moving to Edmonton from out of the area, we kindly request that you seek a primary care provider locally who can then refer you to a pediatrician if specialist care is needed. This form is designed to collect information for patients who already have scheduled appointments with our office. Our staff will guide you on when to complete it. Please be aware that this form cannot be used to sign up as a new patient at our practice. We are currently accepting new patients only through referrals from a physician or Nurse Practitioner. Late and No Show Policy

  

· We ask that you try to arrive at our office 10 minutes early.


·  We have a firm clinic policy around lateness. 

   If you are 10 minutes or more late for your appointment you will likely be rescheduled. 

   This is done in an effort to keep our doctors' schedules running as close to on time as possible.


· We ask that you provide at least 48 hours notice when cancelling an appointment so that this time can be used for other patients.


·  If you do not attend your appointment, cancel within 48 hours, or are more than 10 minutes late, this will be counted as a No Show Appointment. 

For new patients if you have two No Show Appointments your file will be closed. 

For existing patients if you have more than 3 No Show Appointments your file will be closed. Virtual appointments count as appointments in terms of this No Show policy.
